目的 基于贝叶斯理论,采用网状Meta分析法评价不同运动方式对维持性血液透析患者透析充分性的影响。方法 计算机检索Cochrane library、Embase、PubMed、Web of Science、中国知网、中国生物医学数据库、万方、维普等数据库中维持性血液透析患者以运动为干预措施、透析充分性为结局指标的干预性研究。使用Stata 15.0软件和R 4.2.1软件的 Gemtc程序包进行统计学分析。结果 共纳入文献38篇,研究对象1764例,8种运动方式,文献质量为A至B级。与常规透析相比,透析中有氧和阻力混合运动、透析中阻力运动、透析中有氧运动均能改善透析充分性,排序概率显示对透析充分性改善效果最佳的是透析中有氧和阻力混合运动,其次为透析中阻力运动、透析中有氧运动。结论 进行中等强度的透析中阻力和有氧运动可提高透析充分性,应鼓励无运动禁忌的患者进行透析中运动。

Objective To evaluate the effect of different exercises on dialysis adequacy in maintenance hemodialysis patients by using a network Meta-analysis based on Bayesian theory.Methods Embase,PubMed,Cochrane library,Web of Science,China National Knowledge Network,China Biomedical Database,Wanfang Data,and VIP were searched for intervention studies on maintenance hemodialysis patients with exercises as intervention measures and dialysis adequacy as the outcome.The network Meta-analysis was performed using software Stata 15.0 and R 4.2.1 Gemtc package. Results A total of 38 literatures were included,involving 1764 subjects and eight kinds of exercises.The quality of the literature was rated as A or B.Compared with routine hemodialysis,intradialytic aerobic and resistance mixed exercise (IntraME),intradialytic aerobic exercise (IntraAE),intradialytic resistance exercise (IntraRE) can improve dialysis adequacy.The ranking probability showed that the best effect on dialysis adequacy was IntraME,followed by IntraRE and IntraAE.Conclusions Moderate intensity resistance and aerobic exercise can improve dialysis adequacy,and patients without exercise contraindications should be encouraged to perform intradialytic exercise.
