目的 探索基于巡诊服务的安宁共照模式在三级医院的应用效果。方法 在三级医院开展安宁共照巡诊服务模式，并在临床进行实践应用。结果 通过巡诊服务模式为患者提供安宁疗护服务，可有效缓解患者疼痛、改善不适症状、促进舒适，协助未成年人病情告知，帮助患者参与临终决策、完成临终愿望，让患者有尊严地离世。结论 在三级医院开展安宁共照巡诊服务模式具有可行性和现实意义，可推广应用。

Objective To explore the application effect of hospice co-care model based on the mobile medical services in tertiary hospitals.Methods To carry out the hospice co-care mobile medical services in tertiary hospitals and apply it in clinical practice.Results Providing hospice services to patients through the mobile medical services model effectively relieved patients’ pain,improved discomfort symptoms,promoted comfort,assisted in informing minors of their conditions,helped patients participate in end-of-life decision-making,fulfilled end-of-life wishes,and allowed patients to pass away with dignity. Conclusions The hospice co-care mobile medical services model in tertiary hospitals is feasible and of practical significance,which worth promotion and application.
