目的 了解表面麻醉下白内障手术中配合欠佳老年患者的真实感受，为提高该类患者术中配合度及手术安全性提供参考。方法 于2022年9月至2023年6月目的抽样法选取某三级甲等医院眼科表面麻醉下行白内障手术术中配合欠佳的17例老年患者作为研究对象，采用现象学研究法进行半结构化访谈，并使用Colaizzi 7步分析法对数据进行整理和分析。结果 归纳出3个主题：对白内障手术相关知识了解不足；负性心理体验（对手术环境的陌生、手术操作引起的负性心理、担心手术预后）；术中畏光，固视时间久。结论 为了提高老年患者白内障手术的术中配合度，护理人员应完善健康宣教及科普，全方位提高患者对疾病的认知度，并采取有效措施疏导患者情绪，减轻其心理负担，以改善术中配合度，提高手术安全性。

Objective To comprehend the genuine feelings of elderly patients with poor cooperation during cataract surgery under surface anesthesia and to provide references for strategies to enhance the intraoperative collaboration of these patients.Methods From September 2022 to June 2023，17 elderly patients demonstrating poor coordination in cataract surgery under surface anesthesia in the department of ophthalmology in a tertiary A hospital were selected as the subjects.Semi-structured interviews were carried out with the phenomenological research approach，and the data were sorted and analyzed using the Colaizzi 7-step analysis method.Results 3 themes are summarized: deficiency in knowledge regarding cataract surgery; negative psychological experiences (unfamiliarity with the surgical environment，negative psychology induced by the operation，concern about the prognosis of the operation); photophobia and long fixation duration during the operation.Conclusions To improve the intraoperative cooperation of elderly patients undergoing cataract surgery，nursing staff should enhance health education and science popularization，comprehensively raise patients’ awareness of the disease，effectively relieve their tension and psychological burden，and organize preoperative simulation training to enhance intraoperative coordination.
