目的 了解老年痴呆患者照顾者支持性照护需求和外部支持的“赋能差”。 方法 2024年9-11月，目的抽样法选取上海市12名老年痴呆患者照顾者和12名医护人员进行半结构式访谈，基于赋能理论、利用主题分析法和配对分析法对访谈资料进行分析和提炼主题。 结果 共归纳出3个主题：健康照护的迫切需求与专业指导力量薄弱、情感慰藉的深切渴望与支持性干预资源匮乏、社会支持需求的广泛性与供给资源嵌入受阻。 结论 照顾者支持性照护的内部需求强烈，而外部支持不足，可统筹个人、家庭和社会3个维度赋能，提高照顾者支持在社会服务体系中的可见度和优先级，提升老年痴呆患者及照顾者的生活质量 。

Objective To explore the empowering gap between supportive care needs and external support of dementia caregivers.Methods Semi-structured interviews were conducted among 12 dementia caregivers and 12 medical staff in Shanghai from September to November in 2024,with the data analyzed by thematic analysis and paired comparison analysis based on empowerment theory.Results 3 themes were identified:urgent needs for health care vs.insufficient professional guidance,strong desire for emotional comfort vs.lack of supportive intervention resources,common needs for social support vs.impeded resources of supply.Conclusions Dementia caregivers have strong needs,but external support is inadequate.Empowerment through individual,social,and family aspects can enhance the visibility and priority of caregiver support in the social service system,and improve the quality of life of dementia patients and their caregivers.
