目的 汉化和文化调适预立医疗照护计划行为与行动调查问卷（behaviors in advance care planning and actions survey,BACPACS），并评价其信效度。方法 按照新型Brislin翻译模型翻译原文，邀请8名专家参加共识会议，并评价问卷内容效度。采用认知性访谈完成跨文化调适；采用便利抽样法选取171例患者进行调查，检验“您最在意的是什么”维度的信度。结果 共进行3轮认知性访谈，25例患者参与访谈。条目水平内容效度为0.83~1.00；问卷水平的全体一致性内容效度为1.00，平均内容效度为0.917，条目间一致性为0.842。“您最在意的是什么”维度Cronbach's α系数为0.916。中文版BACPACS适用于≥50岁的慢性病患者，包含6个维度，26~38个条目，受访者回答问题的数量取决于分支序列。结论 中文版BACPACS可为预立医疗照护计划相关研究提供工具支持。

Objective To translate and culturally adapt the behaviors in advance care planning and actions survey (BACPACS) and to evaluate its reliability and validity.Methods The original scale was translated following the novel Brislin translation model.8 experts were invited to participate in a consensus meeting and to evaluate the content validity of the scale.Cognitive interviews(CI) were used to complete cross-cultural adaptation.A convenience sampling method was used to survey 171 patients to test the reliability of the “What Matters Most to You” subscale.Results 3 rounds of CI were conducted,involving 25 patients.The item-level content validity index ranged from 0.83 to 1.00.The scale-level universal agreement content validity index was 1.00,the scale-level average content validity index was 0.917,and the inter-item consistency was 0.842.The Cronbach's α coefficient of the “What Matters Most to You” subscale was 0.916.The Chinese version of BACPACS is applicable to patients with chronic diseases aged 50 or more.It consists of 6 dimensions,with 26－38 items,and the number of items answered by respondents depends on the branching sequence.Conclusions The Chinese version of BACPACS can serve as the tool to support research related to advance care planning.
