目的 制订《老年缺血性糖尿病足溃疡患者创面护理专家共识》(以下简称《共识》),规范缺血性糖尿病足溃疡的创面护理流程,为临床医护工作者提供规范化指导。方法 由糖尿病足护理、内分泌、血管外科及康复医学等多领域专业人员共同组建多学科编写团队,经系统文献检索形成《共识》初稿,并通过2轮德尔菲专家函询修订完善。结果 2轮专家函询中,专家积极系数均为100.00%,专家权威系数为0.871,Kendall协调系数分别为0.386和0.549(均P<0.001),根据专家意见对方案内容修改后形成终稿。《共识》围绕老年缺血性糖尿病足溃疡的伤口护理和全程管理的关键环节,推荐建立多学科协作团队、以“蚕食”方式进行清创等。结论 该《共识》科学规范、实用性强,有助于促进伤口愈合,可为老年缺血性糖尿病足溃疡患者的临床创面护理实践提供参考。

Objective To develop the Expert Consensus on Wound Care for Elderly Patients With Ischemic Diabetic Foot Ulcers(IDFUs),standardize the wound care process for IDFUs,and provide standardized guidance for clinical health-care professionals.Methods A multidisciplinary writing team was established,comprising professionals from the fields of diabetic foot nursing,endocrinology,vascular surgery,and rehabilitation medicine.An initial consensus draft was developed through systematic literature retrieval and was revised and refined through two rounds of Delphi expert consultation.Results In the two rounds of expert consultation,the expert response rate was 100.00%,the expert authority coefficient was 0.871,and the Kendall's concordance coefficients were 0.386 and 0.549,respectively(P<0.001).After revisions based on expert opinions,the final version was developed.Focusing on the key aspects of wound care and whole-process management of IDFUs in elderly patients,the Consensus recommends establishing a multidisciplinary collaborative team and performing “nibbling” debridement.Conclusions The Consensus is scientifically sound,standardized,and highly practical.It helps promote wound healing and can provide a reference for clinical wound care practice in elderly patients with IDFUs.
